Subacute

Our Subacute Unit is a 28-bed licensed facility accredited by the Department of Health Care Services (DCHS), specializing in the care of tracheostomy and ventilator-dependent patients. Our facility is uniquely designed to provide life-sustaining support for patients who require complex medical technology, like tracheostomy and ventilator-dependent patients. We strive to maintain a home-like environment that preserves each person’s dignity during their stay with us.

Our social services team assists both residents and their families during transitional periods. We also provide essential information, manage requests and concerns, help in care planning and manage discharge planning for each resident.
